wideband reading rich

I've been trying to figure out what's wrong with my AFR readings. My wideband is reading/logging rich between 10-12.3 afr at all times. I've turned the car on to test the wideband gauge and sensor and it reads 14.7 upon power up like it should and eventually goes full lean like it should. The car runs and drives great, exhaust doesn't have any color or odor to it. My mpg hasn't really changed so i am stumped on what is causing my wideband to read this way. I've ran this wideband sensor/gauge for over a year and the car would hit 14.7 afr all day everyday until i moved the car to the east coast. Sometime during the middle of the car's first winter is when the wideband started reading rich all the time. I am suspecting maybe my wideband sensor needs replaced, possible exhaust leak near the sensor or stock injectors leaking.

anyone ever experience this or have any ideas?

from the logs my o2 Feedback trim (STFT) is between +1.5% and 5%.

Car is a 2005 Evo8 RS, wideband is the AEM Classic Digital wideband with 4.9LSU Bosch sensor.

If you can do a "free air calibration" I'd try that first.

If that doesn't work, replace the sensor.
